The bags themselves are from ClearBags.com: Clear Zipper Pouch Gusset Bag product # ZBG46 (outer dimensions 4 11/16" x 3" x 7 1/4"). 100 pcs for $16.00. (Shipping from here is expensive if you have a small order, but you can get lots of other great stuff here, like organza ribbon, open ended bags for fizzy/tea/chew samples, half sheet bags with adhesive closures for holding folded flyers & RE9 samples, etc. I have smaller zip bags for Evolution Full Control samples, too!)
The labels are Avery # 5264 White Shipping Labels (3 1/3 x 4"), which come 6 on a sheet. Print 3 fronts & 3 backs on a page. The PDF files for the labels will be up in "files" shortly - one for chocolate, one for vanilla. Hope this helps!
